Insurance & Payment Information
We participate with Medicare and many commercial insurance plans. We will submit a claim to the insurance company if we participate with your plan.
All payments are expected at the time of service unless other arrangements are made prior to the visit. We accept cash, check, and Visa or MasterCard.
Some tests and services may not be covered by your insurance plan. You may be asked to sign a waiver by us or the off-site lab performing the tests to ensure payment is received if the insurance company does not cover the service provided.
It is your responsibility to understand how your insurance works. If you need referrals from us prior to seeing other consultants, we will assist you in this process.
A signed financial policy will be required from each patient which outlines the financial agreement and acceptance of the terms.
